A clinic's WhatsApp is almost entirely logistics: book, remind, instruct, report, check in, recall. Every one of those is a utility message, most fire automatically from appointment stages, and together they cut no-shows by a third and take the phone off the front desk. The rules matter here more than anywhere: consent at registration, clinical details kept out of templates and broadcasts, and an AI that never answers symptoms. The first-contact flow

Trigger: any first message, the QR at reception, the website widget. Steps: new or existing patient; department or doctor as a list; date and slot as a WhatsApp Flow form; name and an optional one-line reason (never required); confirmation. Emergency words (pain, bleeding, swelling, emergency) route to a person immediately with the emergency number before anything else. The appointment is created with stage Scheduled and the reminder timers start.

Dental practices add one step: treatment type (check-up, cleaning, filling, root canal, braces review) so the pre-procedure instructions and the recall interval are set correctly. A braces patient recalls every 4 to 6 weeks; a check-up patient every 6 months.

Reports, instructions and check-ins

Pre-procedure instructions go out as a utility template at stage confirmed: fasting for a blood test, no food for two hours before a filling, what to bring. Report-ready messages are utility with a reply button; the report itself travels inside the conversation the patient opens by replying, so clinical content is never in a template or broadcast. Post-visit check-ins at day two or three ("How are you feeling after {{1}}?") route replies to the nurse. Recalls fire at the interval set on the service with slots as buttons.

Collect the phone number and WhatsApp consent at registration, naming the message types; record source and date on the contact. Keep clinical detail out of templates, broadcasts and the AI's knowledge base. Restrict who sees conversations by team (reception, nurses, doctors); the audit log records access; personal fields are encrypted at rest. The security and roles page lists the controls.

The AI, kept to logistics

Upload hours, doctors and specialities, fees, insurance and TPA lists, directions, parking and preparation instructions. Escalation rules: any symptom word, medication question, "emergency", a request for a doctor. The AI introduces itself as handling appointments and information, answers in the patient's language and pauses when a person joins.

Meta policy

Messages about the customer's own booking, order, account or request are utility (Rs 0.115 in India, free inside an open window). Offers, launches and packages are marketing (Rs 0.8631): prior consent, an opt-out line and Meta's per-user frequency cap apply. Promotional wording inside a utility template gets it recategorised and billed at the marketing rate.

The weekly review

Track no-show rate by doctor, reschedule replies and how fast the desk handled them, recall replies (dental recalls at six months typically bring back a fifth of patients within a fortnight), report-ready replies, and escalations from the AI to a person with their reasons. A rising escalation count usually means a knowledge base gap (a new service, a changed fee) rather than a patient problem.

What it costs

A two-dentist practice with 600 appointments a month: confirmation, two reminders and a check-in or report message each (2,400 utility messages, about half free in-window, half at Rs 0.115: Rs 138); 150 recalls (Rs 17); enquiries and reschedules free. Frequently asked questions

Can a dental clinic send reminders on WhatsApp?
